Home Owners Insurance Information

DCI has intervened on behalf of FCC Providers who have received notice of cancellation on their homeowners policy because of doing child care in their home.

DCI has written the Insurance company and included a section of the CA Dept of Insurance Code. This wording can be used for others who may experience the same problem.

Records to be Maintained at the Facility Family Child Care Home The following records are required under Sections of Title 22, California Administrative Code, and must be kept in the facility complete and current, and readily available for review. Go to www.ccld.ca.gov, click onto Licensing Forms.
A.	FACILITY FILE — a copy of each of these forms must be kept on display in the facility:
LIC 203	License ( if your license states 6 or 12 and you take the extra 2school age children, you also need the Family child Care License Addendum)
LIC 610A	Emergency Care and Disaster Plan
LIC 9148	Earthquake Preparedness Checklist (EPC)
	Certification showing completion of Pediatric CPR, Pediatric First Aid, and Preventative Safety & Health
PUB 394	Parents’ Rights Poster - Family Child Care Home (12/06 version)
LIC 9213	Notice of Site Visit and any licensing report documenting a type A citation must remain posted for 30 days after each site visit by a LPA, during the hours that children are in care,
	Any licensing report or other document verifying compliance or non-compliance with DSS order to correct a Type A deficiency must be posted for 30 days during the hours that children are in care.
B.	FACILITY FILE — a copy of each of these forms must be kept on file in the facilityand readily available when a parent asks to see them.

C	CHILDREN’S FILE — a copy of each of these forms must be kept in EACH child’s file:
LIC 700	Identification and Emergency Information for each child enrolled
LIC 627	Consent for Medical Treatment for each child
LIC 282	Affidavit Regarding Liability Insurance signed by each parent, if you are not insured.
LIC 995A	Receipt for Notification of Parent’s Rights signed by each parent. (updated 12/06 version)
LIC 995B	Receipt for Notification Parent’s Rights Addendum to Exclude signed by each parent, if applicable.
LIC 995C	Receipt for Notification Parent’s Rights Addendum to Reinstate signed by each parent, if applicable.
LIC 613	Receipt for Personal Rights signed by each parent.
LIC 9212	Consumer Education Handout, “Family Day Care for Children”, must be available.
PM 286	Blue California School Immunization Record filled out with child’s immunization record
