Reportable Diseases
You and your day care parents should report to each other the diseases in all 3 charts: 
Chart 1

Chicken pox (varicella) Infectious diarrhea Ringworm
Hand, foot, and mouth disease (Coxsackievirus) Lice Roseola
Fifth Disease 
("slap cheek", parvovirus)
Mononucleosis Streptococcal infection 
(including strep throat, scarlet fever)
Herpes Pink eye (conjunctivitis)  
Impetigo Pinworms  
In addition, you should report the diseases in Chart 2 to the local Public Health Department:

Cryptosporidiosis Meningococcal disease Tetanus
Cysticercosis Meningitis Toxic shock syndrome
Dengue Mumps Trichinosis
Diarrhea of the newborn Non-gonococcal urethritis Tuberculosis
Diphtheria Pelvic inflammatory disease (PID) Tularemia
Encephalitis, acute Pertussis (whooping cough) Typhoid fever
Food borne illness Plague Typhus fever
Giardiasis Poliomyelitis Yellow fever
Gonococcal infection Psittacosis
You should also report outbreaks or multiple cases of the following:
